The most important thing you need is your Canadian 'birth certificate'.
With that your Canadian and nobody will deny you a passport.
I would suggest you get in touch with the Canadian embassy branch near you that offers passport services.
Not every consulate is offering that service, but I know Munich is doing that. Just Google for the addresses.
In Canada it doesn't matter if you hold another citizenship beside the Canadian.
I knew a guy who was 'East Indian' , 'British' and 'Canadian' at the same time.
